Complex Healthcare Our Healthcare Services We understand that living with a long-term condition and suffering from a lack of independence can be extremely challenging. You may fear that as your condition progresses, your only option will be to move in to a care facility. However our health services are specially designed to be flexible and adaptable to your ever changing needs, we can support you to remain living in your own home. Working closely with you and your family and any other healthcare professionals involved in your care we can provide Registered Nurses, Healthcare Assistants and Care Workers with the specific skills needed to match your individual complex care requirements. Our complex care is suitable for adults, children and young people with a range of on-going medical and continuing health care needs including: End of life care Palliative care Acquired brain injuries Learning Disabilities support Physical Disabilities support Our Nurses and Carers are able to support with a range of care requirements including, entreal/peg feeding, gastrostomy feeding, stoma care, nasophyaryngeal suctioning, oral suctioning, bowel management, oxygen therapy and tracheotomy care. Rapid Response/Intermediate Care Alongside our Complex Care Service we provide Rapid Response and Intermediate Care designed to prevent admission or to facilitate early discharge from hospital. This service is usually at short notice and as such our team are trained to react quickly in order to provide the care needed. Our Team Our complex care services are managed by our highly experienced Home Care Team and our Lead Nurse. Supporting the implementation of all complex care packages, our lead nurse completes clinical assessments and ensure that all of our clinical tasks are undertaken in accordance with internal policies and NMC guidelines. Our own Quality Assurance Policy and external governance by the Care Quality Commission ensures that a professional, courteous and quality service is provided at all times. What You Can Expect From Us As a valued service user you can be assured that: We will undertake a comprehensive assessment of your needs and complete a full care plan We will provide comprehensive case co-ordination and management of each care plan We will fully manage the discharge process, communicating with all stake holders We will attend all MDT and review meetings We will undertake regular evaluations of the service provided We will work closely with you, your friends and family at all times Your care and support will be provided by fully trained and suitably experienced staff Visits will be made by regular staff who are assigned to deliver your individual care Your personal opinions and preferences will be respected You can also be assured that your individually assigned Nurses or Carers will always: Protect your dignity and privacy Maintain confidentiality regarding your care needs and personal affairs Respect your home and belongings Address you by the name you prefer A Transparent Charging Structure Our working partnerships with Primary Care Trusts and Local Authorities have resulted in a charging structure that is transparent and offers value for money. Every client has an individually costed service and you can expect a full quotation, inclusive of all staffing related costs, before the commencement of any care package. If you are eligible for funding for your care, including Direct Payments or Personal Budgets, we work closely with the Local Authority in order to provide the services required within the rate set.
